The primary location of this position is remote with an expected schedule of Monday - Friday (8:00am-5:00pm).
What You'll Do
The Care Team Assistant Manager, Transitions of Care, will oversee the Care Team Assistants and Care Coordinator roles responsible for supporting our Transition of Care team. This role is largely administrative in that it oversees a non-clinical support team. These roles download and scrub data, prep charts for providers, track caseloads, request records, and provide general administrative support to several teams.
